Improving continuity of care

Practical ways to give clients the same carers more often, and how to tell if it is working.

Why it matters

Clients receiving care at home often say the thing they value most is seeing the same carers. A familiar carer knows the routine, notices changes in someone's health sooner, and builds the trust that personal care depends on.

Record preferences

Ask each client which carers they get on with and keep a ranked list. Preferences change, so revisit the list at reviews.

Plan around them

When building the rota, place preferred carers before filling the rest. Regular patterns help: the same carer for the same visit each week is easier to keep than a new arrangement every Monday.

Measure it

Track the share of visits where the client had one of their preferred carers, week by week. A number you can see is a number you can improve.

Questions

What is continuity of care in home care?
